If you wish to provide questions for your clients to answer in a rating scale, you can achieve this in your custom form builder. A rating scale is a great tool to use when conducting a survey for customer satisfaction/feedback, for example. To begin, click on the form icon at the top of your control panel and choose the option for form editor.
To add rating questions, click on the name of your selected form.
Under the section for miscellaneous, choose the option for rating. You can drag this field above to your desired location in your form.
You can click on the words "Rating Scale" to edit the title. You can also edit the question text by clicking on the prefilled question text (Question 1, Question 2, etc).
You can also modify the rating values (Strongly Agree, Somewhat Agree, etc) by clicking into the text fields and updating the text.
Alternatively, you can click on the pencil icon to the right of the rating scale to choose the advanced option to update this information. You can also add additional rating values in the Advanced section if you want to use more than 5. Typically, an odd number of rating values is recommended for this type of field.
Click save changes to modify your selections if you are using the advanced tool for your rating scale.
